CAN SPAM Law: names are not all pasted into TO: line?

Saw the following on the PDF file about CAN SPAM
under "Email Deployment Checklist:"

Names are not all pasted into TO: line?
(Note: this mistake cost Eli Lilly $160,000)

What does that mean?

Anyone know?

I get some spam with about ten names in the TO: line.
ListMail of course is only used for double opt in mail (by me) but each mail is individual. No one else on the TO: line.
This also keeps privacy of who you send to. They get their own mail, not a circulation list from spammers.
