Need Help Setting Listmail Up

Question- How do i set up Listmailpro like on this site www.doubleyourdating.com .

You need to enter your First name and email and it will automatically take you to the next page.

You can do this from any standard HTML page on your web site.  Once ListMail is set up, browse to the "List Settings" -> "Signup Code" page.  Here you can create some generic HTML you can add to your web site.  You'll probably want to format it further to match your design.
