How to move my account, lists and databases to a new server?

I am having trouble with my hosting company and want to move my account, lists and databases to a new company.  I don't see anywhere were instructions to this are posted.  Can you help

PS: Is it 1 listmail account per domain, or may I use list from 2 or 3 domains?

At this time we allow you to install the program on more than one of your own web sites.  Of course, if another company or administrator will be using the program we expect you to refer them to us.  If/when we implement site licensing we will compensate current owners generously with up to 5 licenses.

Basically it's a 4 step process:

1) Set up a new MySQL database, username and password on the new host
2) Upload the ListMail files
3) Restore a ListMail backup or manually import your previous MySQL database
4) Configure ListMail for the new host

There are a few useful posts such as this one and this one - try searching the forum for "manual backup" (using the "All terms" option) or just "backup".
